How to Contact Us
To obtain more information on Toronto's Food Premises Inspection and Disclosure System, you can contact us by email or phone:

Two screens are provided:
- General Comment - use this screen if you have a general comment or question that is not related to a specific Food Premises.

- Food Premises Complaint - use this screen if you have a complaint about a particular food premises. To do this, first find the Food Premises by conducting a search.

The information you provide on either screen is secure and can be submitted without identifying yourself. However, should you wish to receive a reply, a Public Health Inspector will respond when you provide either:

Toronto residents who suspect that they have food poisoning after eating in a Toronto food
establishment should consult with their physician and report the illness as soon as possible to
Toronto Public Health's Communicable Diseases Surveillance Unit at (416)392-7411.
Food poisonings SHOULD NOT BE reported using the Dine Safe web site.
If you live outside of the City of Toronto please report the illness to your local health
department (for a listing of all Ontario health units and their contact information
please click here).
Regardless of where you live, all other complaints related to a Toronto food establishment
can be made using the Dine Safe website by conducting a search at first.
